The request form for the COVID-19 vaccine incentives is now live.
Minnesotans 12 and older who have gotten their first COVID-19 shot since May 27 can fill out the form and choose their preferred reward.
Incentives are only available for the first 100,000 requests.
Rewards include a free state park pass, fishing license, and a $25 Visa card, as well as tickets for Valley Fair, the State Fair, and baseball games.
Sanford Bemidji is now offering COVID-19 vaccines to community members at their Walk-In Clinic.
